FINAL JUDGMENT
This action was heard after entry of Summary Judgment as to Count II of Plaintiff’s Complaint against defendant and the court finds that Plaintiff is entitled to an itemized specification of unpaid charges from Defendant (as set forth in Florida Statute s. 627.736(4)(b)) and that the Defendant has failed to furnish said itemized specification.
IT IS ADJUDGED that plaintiff R. J. Trapana, M.D., P.A. recover from defendant United Automobile Insurance Company an itemized specification of unpaid charges as set forth in Florida Statute s. 627.736(4)(b) (detailing each of the charges received by Defendant from Plaintiff and each reason for non-payment (or reduced payment) of each unpaid or partially paid charge.
Plaintiff, R.J. Trapana, M.D., P.A. is also awarded reasonable attorney’s fees and costs against Defendant in an amount to be determined at a later hearing.
