STATE FARM MUTUAL AUTOMOBILE INSURANCE COMPANY, Plaintiff, vs. SPEEDY GLASS AND WINDOW, INC., Defendant. County Court, 11th Judicial Circuit in and for Dade County, Civil Division. Case No. 07-05067-CC-05 (6). August 1, 2007. Bronwyn C. Miller, Judge. Counsel: David B. Kampf, Ramey & Kampf, P.A., Tampa, for Plaintiff.

FINAL JUDGMENT

THIS CAUSE came before the Court on Plaintiff, STATE FARM MUTUAL AUTOMOBILE INSURANCE COMPANY’s Motion for Entry of Final Judgment, and the Court, having heard argument and being otherwise advised in the premise it is hereby:

ORDERED AND ADJUDGED:

1. State Farm shall be entitled to an award of damages from Defendant totaling $4,199.20.

2. State Farm shall be entitled to an award of costs from Defendant totaling $245.00.

3. State Farm shall be entitled to an award of pre-judgment interest at 9% per annum from August 1, 2006. Pre-judgment interest shall total $377.93, which is one full year of interest.

4. Therefore, this Court finds that State Farm is entitled to recover from Defendant the total sum of $4,822.13 that shall accrue interest at the rate of 11% per annum, for which sum let execution issue.

5. It is further ordered and adjudged that the judgment debtor, SPEEDY GLASS AND WINDOW, INC., and its President, Jose Alfonso, shall complete under oath Florida Rule of Civil Procedure Form 1.977 (Fact Information Sheet), including all required attachments, and serve it on the judgment creditor’s attorney, Ramey & Kampf, P.A., within 45 days from the date of this Final Judgment, unless the final judgment is satisfied or post-judgment discovery is stayed.

6. Jurisdiction of this case is retained to enter further orders that are proper to compel the debtor, SPEEDY GLASS AND WINDOW, INC., and its President, Jose Alfonso, to complete form 1.977, including all required attachments, and serve it on Ramey & Kampf, P.A. This Court will reserve jurisdiction to address claims against Jose Alfonso, in the individual capacity pending compliance with this Final Judgment.
