ORDER

THIS MATTER having come before this Honorable Court on Plaintiff’s Motion to Enforce Court Order Executed on July 9, 2013 and Motion for Sanctions againstdDefendant, Auto-Owners Insurance Company for Failure to Comply with Court Order Executed On July 9, 2013 and this Honorable Court having heard arguments of counsel on September 12, 2013 and being otherwise fully advised in the premises, it is hereby,

ORDERED AND ADJUDGED that:

1. Plaintiff’s Motion to Enforce Court Order Executed on July 9, 2013 and Motion For Sanctions against Defendant, Auto-Owners Insurance Company For Failure To Comply with Court Order executed on July 9, 2013 is hereby GRANTED.

2. Defendant shall comply with the Court’s July 9, 2013 Order, paragraph numbers four (4) and five (5), by producing to Plaintiff the Fort Lauderdale Fire medical bill in this claim and the EOR for said medical bill.

3. Defendant shall comply with the Court’s July 9, 2013 Order, paragraph number twelve (12), if no IRS 1099 forms were sent from Defendant to Corvel in 2012, Defendant shall produce the documentation that indicates what amount Defendant paid Corvel in 2012.

4. Defendant shall comply with the Court’s July 9, 2013 Order, paragraph number thirteen (13).

5. Defendant shall provide the documents set forth in paragraph numbers two (2), three (3), and four (4) above within sixty (60) days of the September 12, 2013 hearing.

6. Plaintiff is permitted to take the deposition of Defendant’s adjuster Theresa Sterling again in this matter. The deposition of Theresa Sterling shall be coordinated within forty-five (45) days and shall occur within one hundred and twenty (120) days of the date of the September 12, 2013 hearing. Defendant shall pay for the costs of said deposition.
