THIS CAUSE having come before the Court on Defendant’s Motion for Protective Order as to Electronic Discovery and Insurer’s Claims File and Business Practices and the Court having heard argument from Counsel, it is hereupon,

ORDERED AND ADJUDGED that:

IN RESPONSE TO PLAINTIFF’S REQUEST FOR ELECTRONICALLY STORED INFORMATION NO. 8 — DEFENDANT SHALL PRODUCE A TABLE OR TABLES OF THE CHARGES/BILLS RECEIVED BY THE DEFENDANT AND/OR ITS SUBSIDIARIES AND/OR SISTER COMPANIES FOR CPT CODES 72040, 72070, 72100, Q0092, R0070, FOR ANY AND ALL MEDICAL PROVIDERS THAT RENDERED SAID SERVICES (RELATIVE TO THE ABOVE-REFERENCED CPT CODES) AND SUBMITTED A BILL(S)/CHARGE(S) FOR SERVICES RENDERED IN MIAMI-DADE COUNTY BETWEEN JULY 23, 2011 AND SEPTEMBER 23, 2011 TO THE DEFENDANT AND/OR ITS SUBSIDIARIES AND/OR SISTER COMPANIES, TO BE PRODUCED WITHIN THIRTY (30) DAYS OF THIS ORDER.

THIS/THESE TABLE(S) SHALL INCLUDE THE FOLLOWING INFORMATION:

1. THE CPT CODE;

2. THE NAME OF THE MEDICAL PROVIDER THAT RENDERED THE SERVICES;

3. THE MEDICAL PROVIDER NPI;

4. THE AMOUNT CHARGED/BILLED BY THE MEDICAL PROVIDER; AND,

5. THE NUMBER OF UNITS BILLED.

THIS/THESE TABLE(S) SHALL BE PROVIDED IN A REASONABLY USEABLE FORMAT, INCLUDING .CSV, .TSV, .XML, OR MICROSOFT EXCEL SPREADSHEET (.XLS/.XLSX).

PLAINTIFF’S REQUEST FOR ELECTRONICALLY STORED INFORMATION NOS. 1-7, AND 9-12 ARE HEREBY WITHDRAWN.
