ORDER ON PLAINTIFF’S FIRSTMOTION FOR JUDICIAL NOTICE
THIS CAUSE having come on to be heard (May 21, 2015) on Plaintiff, FLORIDA INJURY & REHABILITATION CENTERS, INC., a/a/o Felipe Cardona’s Motion for Judicial Notice, the Court having been otherwise fully advised in the premises and by review of the file, record and authorities, it is hereby,
ORDERED AND ADJUDGED:
1. Plaintiff’s First Motion for Judicial Notice is GRANTED as set forth herein, including page 30 of 246 and page 190 of 246 of the National Uniform Billing Committee Official UB-04 Data Specifications Manual.
2. The Court hereby takes judicial notice of Form Locator 14 of the UB-04 claims form. Form Locator 14 definition is “A code indicating the priority of this admission/visit.” Code “1” is “Emergency” which definition is “The patient requires immediate medical intervention as a result of severe, life threatening or potentially disabling conditions.”
3. The Court hereby takes judicial notice of Form Locator 76 of the UB-04 claims form submitted. Form Locator 76 definition is “The Attending Provider is the individual who has overall responsibility for the patient’s care and treatment reported in this claim.”
