In fact, they are the most undiagnosed and underreported injury after a car accident. Hospitals miss them all the time. It is important to have each of your patients fill out this form on their first visit at your office so that you can properly document if they had any symptoms of a possible brain injury/concussion.
Obviously it is important to properly evaluate your patient for their health. Also, having the patient complete this form will protect you from claims that you did not properly diagnose the patient’s head trauma.
