SOAP Note For Manual Therapy

I have received multiple phone calls and emails from doctors who read my blog. They want to know what they can put in their SOAP notes to describe manual therapy. These are mostly doctors who didn’t previously bill/perform manual therapy. Here are some suggestions:

  • Manual therapy in the form of soft tissue mobilization was performed on the INSERT AREAS OF BODY (ie. left cervical, right trapezius, upper thoracic, mid thoracic, left lower thoracic, lumbar, right lumbar, left sacroiliac, etc) region(s) for X minutes.
  • Manual therapy in the form of trigger point therapy was performed on the INSERT AREAS OF BODY (ie. left cervical, right cervical, left trapezius, and right trapezius) region(s) for X minutes.
  • Manual therapy in the form of manual traction was performed on the INSERT AREAS OF BODY for X minutes.
  • Manual therapy in the form of myofascial release was performed on the INSERT AREAS OF BODY (ie. left cervical, right cervical, left trapezius, and right trapezius) region(s) for X minutes.
